I am planning to depart Switzerland from Zermatt and enter Italy via Milan.

I understand the train route should be Zermatt - Brig - Milan.

I have a Swiss Travel Pass - is there any additional ticket that I need to purchase?

Yes. The Swiss Travel Pass is valid as far as Domodossola.

You need a ticket plus seat reservation for Domodossola-Milan.
