I bought separate tickets from Spiez to Demodossola (the border of Italy) and Demodossola to Milano Centrale, but the train actually goes directly from Spiez to Milano Centrale. Do I need to get off in Demodossola for someone to check the ticket or I can sit all the way through?

You mean Domodossola.

No, you can remain on board. Custom officers may get on board the train and ask for your documents and possible goods to declare.

You can stay on the train. Your ticket from Domodossola to Milan will include a seat reservation. You can therefore use this seat from Domodossola at the latest, or from Spiez if it is free.
