See the SBB ticket shop: https://www.sbb.ch/en. Simply enter from Zurich HB to Zermatt and choose a ticket.

2)
The SBB timetable is not proposing connections which are by default not doable. 5 minutes is sufficient, and you will not be the only one doing this transfer.
