For example: train departs from Lausanne at 19:18 and arrives at 22:40 in Milan, Italy. Will I need to purchase a ticket?

Your Seven25 abo is valid until Domodossola, if you leave Lausanne after 19.00. You have to buy a ticket from Domodossola to Milano Centrale. You do not have any additional discount. You will find more information here.
